Ready to get that perfect puppy? Great! There are few things that we both need to do.
First, you need to fill out an application. Why do we have an application? Because Cotons are a special breed, they need to be placed with special families. The application lets us know about you, and you get to answer some questions that you should be asking yourselves in the first place.
After you fill it out, send it to Barbara at bbdavis01@hotmail.com as an attachment. Or you can fax it to 304-343-4113 attn Bruce Davis. Or you can snail mail it. Call Barbara at 304-757-7369 to get the address.
Two ways to get the application form: - 1) has three options, 2) is easy but takes a wee bit longer.
1a) Download a Word Document here. It will load into an Explorer window, and you can save it from there.
1b) Download an Acrobat PDF form which you can fill out here. You need Adobe Acrobat Pro ($$$) or Adobe Acrobat Reader and Adobe Air for this form. Download Reader and Air for free here. (I couldn't get Air to install right). Without Air, you can still fill it out and print it, you just can't save it (but that's the point isn't it?)
1c) Download a plain old Acrobat PDF application here.
2) Alternately, just email Barbara and request the application form, and she will email or fax it to you it to you quick like a rabbit.
Then if we both agree that you are the perfect family for a puppy, you get to sign a contract and send in a deposit (oh, goodie). The contract is not online yet, but will be soon.
